It is essential to keep track of a patient’s medical history in order to identify a patient’s possible needs and restrictions in terms of medication and other medical procedures. The City Government of Marikina tries to address this matter through the Family Health Book (FHB).

Program goals/rationale: 

The FHB tries to promote health consciousness among the residents of Marikina City.

Key program components: 

The FHB contains records of medical consultations, immunizations, and laboratory procedures for all members of the family, as well as a substantial number of health tips.

Program history: 

The city’s “Health Passport: Your Guide to Healthy Living” program is implemented over a population of 430,000 people. It is a well-established program run by the City Health Office.
