Summary: 

Srikon Healthcare Services is a home-based primary care organization that provides integrated care in India.

Program goals/rationale: 

The need for such a programme was felt as the growth of multiple specialties and an increasing number of care providers and options often leave the patient confused.

Key program components: 

Under the program, a family or individuals can subscribe to Srikon services, ranging from routine doctor's check-up to super-specialty advisory services. Srikon Healthcare has in-house doctors, who will be individually assigned for specific patients. The family physician will assess and treat ailments in the context of the family and their environment rather than affording treatment in isolation. The readings and the physician's remarks are noted in the patient's file, which gets updated in the automated records.

The company offers to two packages for members. Depending on the need, the beneficiary can choose to have a general services or a customized package. The amount of the benefit package varies from INR 8000 (USD 150) to INR 10000 (USD 188) annually. The two available packages are further detailed below.

Package A - General
-6 house visits (3 compulsory visits, every 4 months)
-Regular updates after every house visit
-Consultation for routine medical requirements or for advice
-Medical Counseling - Educating and helping people to cope with their medical problems, making them understand their present medical condition (if any)
-Specialist referral - Referring to the concerned specialist if required after initial work up
-24/7 assistance on phone for any health related query
-Primary-care consultation - Treating and prescribing medicines wherever required
-Home delivery of monthly refills for people on regular medication
-Facilitation of sample collection
-Patients are informed of their regular investigations and follow up visits with their specialist
-Liaise with the patient’s specialist wherever required
-Guidance in hospitalization wherever possible
-Diet and nutrition planning as required

Package B - Customized package: This package is designed to suit independent needs of a member. Clients who have bedridden members will be assessed at by the visiting doctor. The cost of the package will depend on visit by doctors, physiotherapists, dieticians etc., providing refills, conducting investigations etc. Medical counseling will be provided by the doctor depending on the requirement of the member and the caretaker.
